Holds your slot, but you will not be charged today. Provide at least 3 days' advance notice for any cancellations or changes. Cancellations made with less than 3 days' notice or missed appointments are subject to a late cancellation / no-show fee charged to the card on file.
Health history questionnaire
If relevant to your package & partner provider, we will email you a confidential Personal & Health Information Questionnaire and ask for a copy of your ID. The hospital needs this information to prep your visit safely.
