Health history questionnaire
If relevant to your package & partner provider, we will email you a confidential Personal & Health Information Questionnaire and ask for a copy of your ID. The hospital needs this information to prep your visit safely.

How Payment Works
Depending on your selected package and provider, payment will either be collected directly or you will receive a secure payment link via email prior to your service.
