Fatty Liver Is the Most Common Disease Nobody Knows They Have

About 30% of adults worldwide are walking around with fat accumulating in their liver — a pooled prevalence of 30.05% (95% CI 27.88–32.32) across 92 population-based studies covering 9.3 million people — and the overwhelming majority have no idea. There is no pain, no fatigue you would attribute to it, no visible sign. The liver has no pain nerves in its interior and enormous spare capacity, so it degrades silently for years before anything feels wrong. By the time symptoms appear, the easy-to-reverse window has often closed.

That combination — extremely common, completely silent, reversible if caught early — makes fatty liver one of the clearest examples of what health screening is actually for.

Why Was NAFLD Renamed MASLD?

You may know the condition as NAFLD — non-alcoholic fatty liver disease. In 2023 a multisociety Delphi panel of 236 experts and patient advocates from 56 countries, led by three pan-national liver associations, replaced the term with MASLD: metabolic dysfunction-associated steatotic liver disease. Two-thirds of that panel (66%) found the word "fatty" stigmatising, and 61% said the same of "nonalcoholic".

The rename matters for the same reason the recent PCOS-to-PMOS rename did: the old name defined the disease by what it isn't (not alcohol) instead of what it is. The new definition requires at least one of five cardiometabolic risk factors to be present — so MASLD is, by definition, a metabolic condition, tightly bound to insulin resistance, visceral fat, type 2 diabetes, and cardiovascular risk. The same consensus created a separate category, MetALD, for people who have MASLD and also drink 140–350 g of alcohol per week (women) or 210–420 g (men).

Why Does a Fatty Liver Matter?

Fat in the liver is not inert storage. In a subset of people, it drives ongoing inflammation (steatohepatitis, or MASH), which scars the liver year after year. The progression runs: simple fatty liver → inflammation → fibrosis → cirrhosis, with liver cancer risk rising along the way.

  • Progression is slow — and much slower without inflammation. Paired-biopsy cohorts put fibrosis progression at roughly one stage per 7 years in people with steatohepatitis, versus one stage per 14 years in those with simple fatty liver. Screening is not about declaring an emergency; it is about finding the minority who are progressing while intervention is still easy.
  • The heart is the usual cause of death, not the liver. In the global meta-analysis, cardiac-specific mortality ran 4.20 deaths per 1,000 person-years against 0.92 for liver-specific mortality. A fatty liver is a loud metabolic alarm bell for the whole body — finding it should trigger a look at your heart and metabolic picture too.
  • Across a population, the liver damage still adds up. MASH-related cirrhosis is already the leading indication for liver transplantation in women and in people over 65, and is on par with alcohol as the leading indication overall.

Can Normal Blood Tests Miss Fatty Liver?

Yes — and in two different ways. First, the reference range most laboratories print is too wide: AASLD puts a true normal ALT at 29–33 U/L in men and 19–25 U/L in women, well below the cut-off many labs flag on. Second, on imaging, conventional B-mode ultrasound lacks sensitivity for lesser degrees of steatosis, particularly in people carrying extra weight — and the absence of visible fat on ultrasound does not exclude steatohepatitis or fibrosis. Symptoms arrive only at the late stages, if at all.

Does Fatty Liver Reverse?

Unlike most chronic disease trajectories, early-stage MASLD is meaningfully reversible, and the interventions are known:

  • Weight loss is the primary treatment. AASLD's guidance: losing 3–5% of body weight improves steatosis, while more than 10% is generally required to improve steatohepatitis and fibrosis.
  • Exercise works independently of the scale. Regular exercise has hepatic and cardiometabolic benefit even without weight loss — the same pattern we described for visceral fat, and for the same metabolic reasons.
  • Cut sugary drinks specifically. Excess fructose raises the risk of MASLD, steatohepatitis and advanced fibrosis independently of total calorie intake.
  • Alcohol still counts. The "metabolic" rename doesn't make alcohol safe. AASLD classes intake up to 20 g/day (women) and 30 g/day (men) as mild; heavy drinking accelerates fibrosis, and obesity plus alcohol act synergistically on the risk of cirrhosis and liver cancer.
  • Coffee, oddly, appears protective. AASLD says three or more cups a day can be recommended absent contraindications, on the strength of epidemiological studies and meta-analyses showing reduced risk of fatty liver and fibrosis.

Who Should Actively Look for It?

AASLD directs fibrosis screening at people with type 2 diabetes, medically complicated obesity, a family history of cirrhosis, or more than mild alcohol consumption. Diabetes is the strongest signal of all: reported MASLD prevalence in people with type 2 diabetes ranges from 30% to 75%. Anyone with steatosis on imaging, or with two or more metabolic risk factors, should get a FIB-4 as the first risk assessment — repeated every 1–2 years in prediabetes and type 2 diabetes.

A silent, common, reversible disease is exactly the kind screening earns its keep on. The liver won't tell you — so at some point, it's worth asking.
